Active Property Management NH ("the Company") may obtain "consumer reports" or "investigative consumer reports" about you from a consumer reporting agency for employment or contractor purposes.
A "consumer report" is a background screening report that may contain information regarding your criminal history, sex offender registry status, employment history, education history, driving history, professional licenses, and other information about you. It may bear upon your character, general reputation, personal characteristics, and/or mode of living.
An "investigative consumer report" is a special type of report that includes information regarding your character, general reputation, personal characteristics, and mode of living, which may be obtained through personal interviews with individuals who know you, such as previous employers or references. You have the right, upon written request made within a reasonable time after receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report.
acknowledge receipt of the separate document entitled Disclosure Regarding Background Investigation, and I confirm that I have been provided access to the official federal document entitled A Summary of Your Rights Under the Fair Credit Reporting Act.

I hereby authorize Active Property Management NH ("the Company") to obtain "consumer reports" and/or "investigative consumer reports" about me for employment or contractor purposes at any time during the hiring process and throughout my employment/engagement, to the extent permitted by law.
I understand that such reports will be prepared by the Company's designated consumer reporting agency.
To enable the consumer reporting agency to complete the reports about me for the Company, I hereby authorize any law enforcement agency, administrator, state or federal agency, institution, school or university (public or private), information service bureau, past or present employer, or other party to furnish information about me to the Company and its designated consumer reporting agency.
I further agree that the Company and its designated consumer reporting agency may communicate with me by email or phone regarding issues related to the reports.
